Confidentiality and Non-Disclosure Agreement

All Lehman College students completing Fieldwork or Student Teaching/Intern requirements will be required to digitally sign the following Confidentiality and Non-Disclosure Agreement in an online form when receiving assistance from the Office of Clinical Practice and Partnerships.

I, [the student name and EMPLID typed digitally in an online form], an aspiring teacher completing NYSED fieldwork hours or student teaching requirements at a local school, understand and agree to the following conditions regarding the confidentiality of all people and information encountered during my school/classroom experiences:

Obligation to Maintain Confidentiality

- All information related to students, their families, and school employees must be kept strictly confidential. The student given fieldwork privileges in a school has an ethical and legal duty to do so.
- This obligation applies to all information, whether written, verbal, or electronic.
- This includes student academic performance (grades, test scores), discipline records, health information, disabilities, family situations, and any personal information shared in confidence.

Handling Confidential Information for Required Coursework

- Conversations with the university supervisor, cooperating teacher, or other school officials should take place in a private setting.
- When reporting on students for the university program or coursework, pseudonyms must be used, and identifying information should not be used.
- Classroom teaching videos for educative purposes can only be shared in a secure Lehman College approved system.

Handling Confidential Information

- Confidential information must not be discussed in public settings, including hallways, teachers' lounges, etc.
- Confidential information (including teaching video) cannot be shared on social media, online forums, or any personal or public accounts.
- Confidential information should only be accessed when necessary for professional duties.
- Unauthorized photocopies or electronic copies of student records are not permitted.
